Rational Approach to the Patient with Dyspepsia

Sažetak

The optimal diagnostic approach to the dyspeptic
patient is still debated. Early endoscopy continues to
be the diagnostic gold standard but competing non-invasive
diagnostic strategies challenge this. Early endoscopy is recommended
in patients with alarm symptoms and in patients
with new onset dyspepsia after age 50. The test-and-treat, the
test-and-scope and the empiric antisecretory treatment are the
most important approaches today but the fi rst one is more costeffective
than others if the cost of upper endoscopy is more
than 500 US $. The test-and-treat strategy is superior to the
empiric antisecretory treatment in younger dyspeptics without
alarm symptoms. The urea breath and the antigen stool tests
are recommended for Helicobacter pylori testing. It is needed
to determine the age treshold for our population regarding diagnostic
and therapeutic recommendations for dyspeptics using
(real!) costs of endoscopy and H. pylori testing in Croatia.
