Noninvasive ventilation in chronic obstructive pulmonary disease patient

Sažetak

The use of non-invasive ventilation (NIV) has dramatically increased over the last decade in the treatment of acute respiratory failure (ARF) due to various causes. In acute exacerbation of chronic obstructive pulmonary disease (COPD) it has been shown to significantly decrease partial pressure of carbon dioxide, increase pH, decrease respiratory rate, and diminish the severity of breathlessness. Additionally, it is associated with a marked reduction of the need for endotracheal intubation, reduction of complication rate, duration of hospital stay and hospital mortality. NIV can be useful in facilitating extubation of COPD patients to avoid the
complications of prolonged invasive ventilation. Also, NIV is a useful tool for the prevention and treatment of extubation failure in high risk patients. The studies of NIV in stable COPD patients with respiratory failure have not hown a significant impact on survival, and long term oxygen therapy remains the gold standard. NIV may be delivered either by critical care ventilators or portable ventilators. Studies failed to show significant differences in efficacy between different ventilator modes. The major determinant of NIV overall success is the
choice of interface. When initializing NIV, patients should be closely monitored by trained and experienced staff, and attention should be paid to vital signs, gas exchange and tolerance of the interface. Patients who do not respond adequatel to NIV should be considered for immediate intubation. Since NIV is an effective tool in
the treatment of respiratory failure in COPD, every involved clinician should be familiar with this technique.
