Sažetak

The German model of oral prophylaxis has experienced several structural changes in the past 20 years. The reason for these changes being the unsatisfactory results, as a consequence of lack of good prevention of caries lesions in children and youths. Several cities and only two countries, fulfil the standard WHO regulations of a maximum of 3 DMF-T teeth per child, or 50% of 5/6 year old children with caries- -free dentition, respectively. According to recent studies oral prophylactic measures, introduced by law in 1989, did not succeed: only 4.7% of the pre-school children and 6.5% of the school children up to the age of 12 participated in the fluoridization program, as the most efficient and effective program in the prevention of caries. Despite the poor success oral prophylaxis in Germany has shown a continued reduction of caries so far; for about 70% of the population utilizing the various preventive and treatment concepts. This has been the result of the mutual work of health authorities and dentists employed in private practices. Consequently the health insurance institutions have suggested a new concept of group porohylaxis, comprising primary and intensive measures carried by out professional teams of prophylaxis, aiming at the effective prevention of caries lesions in all social classes with maximum participation of children and youths.
