Early recovery of the patients after the mini-invasive total hip and after the classical surgical approach; preliminary data

Sažetak

Objective: Minimally invasive approaches for total hip arthroplasty (MIS) have created
significant interest of patients and physicians. The assumption is that the MIS approach reduces
postoperative functional recovery due to reduced muscle damage. This study evaluates the clinical
functional recovery in patients who had total hip replacement performed using MIS anterolateral
approach or the conventional lateral approach. Subjects and methods: In two groups of 64
patients were analyzed flexibility, VAS pain scale, walking speed at 50m and abduction strength
7 days and 3 months postoperatively. Preoperatively, there were no differences between the
groups. Results: Differences in indicators of postoperative functional recovery were found in the
strength of abduction after seven days and three months in favor of the MIS group patients
(7th day P < 0.001, at 3 months P < 0.001) and walking speed at 50 meters just after 3 months
(day 7 P = 0.509, and after 3 months P < 0.001). Patients who underwent MIS method had better
flexibility in the hip which is shown in better passive and active flexion (P < 0.001 seventh day
and after 3 months) and felt less pain after 3 months (P = 0.205 after 7 days and P < 0.001 at
3 months). Conclusion: Early recovery in the group of patients treated with MIS method was
faster than in the group of patients treated by conventional method. Based on early results, we
recommend the MIS approach as the standard for total hip arthroplasty, but it takes a longer
period of monitoring to be able to make any final conclusions.
