PSYCHIATRIC APPROACH TO THE PATIENTS WITH MALIGNANT DISEASE IN TERMINAL PHASE

Sažetak
Terminal phase of the malignant disease is a term
that refers to the advanced stage, when malignancy
with metastases has developed to such an extent
that it is realistic to expect the approaching fatal
outcome. The aim of this paper is to discuss from
a psychological point of view, topics related to the
terminal phase of the disease, the patient’s death and
his loss to the family. The main focus of our work,
as a consultative psychiatry (liaison psychiatry) is
to demystify the above mentioned topics and to
shed light on the psychological dimension of the
various problems that disease with the inevitable
fatal outcome may cause. It would be wrong, both
professionally and morally, to ignore the needs of
those patients who failed to achieve the primary
objectives, i.e. complete healing. For all the doctors,
a new target has to be a more humane approach to
the control of terminal stages. With this review
we have tried to clarify our viewpoint about these
ethical issues because we want to encourage other
colleagues to sharpen their ethical sense, as ethics is
a basic need for physicians. Therefore, one should
remember the saying that it is not enough just to
have conscience or empathy, it is also necessary
that it wakes up and burns.
