Are there differences in the organizational structure and functioning of family practice between Krapinsko-zagorska county and Republic Croatia?

Aim. To determine the differences in family medicine (FM) organization in Krapinskozagorska County (KZC) compared to Croatia and
the possible reasons. Methods. The data were collected from the
Croatian Health Service Yearbooks 1995-2014, the Croatian network of planned and contracted family practices (FP), and national census 2011.
Results. There is no lack of FPs in KZC while in Croatia missing are 123 FPs. Percentage of FM specialists in KZC was always below the Croatian
average; the difference was highest in 2014 when there were 30.3% FM specialists in KZC and 48.9% in Croatia. The average number of patients per FP is less in KZC than in Croatia. The number of visits
per patient increased by 134.5% in KZC and by 84.7% in Croatia, and the number of visits per FP increased by 98.1% and 84.6%, respectively. The average number of check-ups in FP decreased by
28.1% in KZC and by 42.2% in Croatia, and the referral rate decreased by 24.0% vs. 34.0%. Conclusion. During the study period the average
patients’ number per FP decreased while the FM usage was higher in KZC than in Croatia. There were more primary care doctors per 10.000
inhabitants and no lack of FPs in KZC. However, these physicians are not properly distributed in KZC and the share of FM specialists is below the
Croatian mean. The solution lies in comprehensive, continuous specialist training in FM and in planning of FM network according to the needs.
