Sažetak
The current concepts are reviewed of the immunologic differentiation of bronchopulmonary allergic affections, based upon the principles of a new classification of allergic reactions which distinguishes the four types of reaction according to Coombs and Gell. The association of bronchial allergic reactions with type I (reaginic-mediated) allergy and of alveolar allergic reactions with type III (Arthus, precipitin-mediated) allergy is described in details. Allergic alveolitis is presented as a pathogenetic entity comprising a number of etiologically different diseases which are almost invariably of occupational origin. All available literature data are given concerning twelve etiological variants of allergic alveolitis (farmer's lung, maple-bark disease, bagassosis, suberosis, mushroom-worker's lung, bird-fancier's lung, pituitary snuff-taker's lung, lung disease due to grain weevil, sequoiosis, malt-workers' lung, cheese-washers' lung, detergent disease of the lung). The variant known as farmer's lung is extensively reviewed not only because of its huge importance in clinical medicine in general and in occupational medicine in particular, but also as a pathogenetic model for all other variants.
