Sažetak

Today, ESWL is the standard method in the minimum invasive treatment of urolithiasisis and is performed at every urological clinic or department as a prymary procedure in the therapy of urinary stones.
The efficacy data on the latest lithotriptors are well known, the results of the treatment of renal and urethral stones presentd many times and the adverse events in laboratory animals and clinical series well documented. The question, however, remains - how come a particular stone is disintegrated easily by shock waves if it is a struvite one, but only into rough fragments or not at all if it is a cystine or calcium oxalate one? Which spectrum frequencies produced by shock-wave generators are considered responsible for stone disintegration and which ones for tissue trauma? These questions form the basis of the research, the results of which are not yet known. Where are the long term results of large-scale series confirming that ESWL does not impair renal function nor cause hypertension? Due to increasing cost of treatment and assuming equal efficacy, should ureteroscopic treatment of ureteral stones be preferred to ESWL? Who takes care of the patients after ESWL treatment until they are stone free and are the patients with recurrent urolithiasis included into a complete programme of metaphylaxis? These are only a few issues necessitating further research of the application of this nowadays-standard method of urolithiasis treatment. Recently new application possibilities for this method have been developed in the treatment of cholelithiasis and sialolithiasis. The latest successes of the shock wave application have been observed in the treatment of pseudoarthroses and tendopathies on various sites.
