Compressive neuropathy of the tibial nerve as a cause of foot pain

Foot pain is a common symptom in out-patient polyclinics. Tibial nerve compresive neuropathy in the tarsal tunnel is rarely diagnosed as a cause of foot pain. It is among the rare compressive neuropathies and “entrapment” syndromes. The aim of this study was to determine the frequency and characteristics of tibial compressive neuropathy in patients with foot pain. The study was conducted in out-patient polyclinics specialized in physical medicine, rehabilitation and rheumatology, on the sample of 10,000 patients over a period of 12 months. There were 43 patients suspected with tarsal tunnel syndrome. Tibial nerve neuropathy was confirmed in 21 patients (48.8%), while other patients had signs of radiculopathies, polyneuropathy or had normal findings. Inflammatory rheumatic disease, foot and ankle trauma and degenerative changes of the arch of the foot and ankle were the most prominent causes of compressive neuropathies. The
results of the study, with incidence of 0.0021% patients in total sample, shows that despite of the low number of patients with this type of compressive neuropathy, it should be considered in the differential diagnosis of the foot pain.
