PERINATAL OUTCOME OF PREECLAMPSIA: CAESAREAN SECTION AND TRACHEOSTOMY – CASE REPORT

Sažetak
The most of anestesiology complications are described in connection with urgent cesarean sections indicated because of the threat for the health of the mother or a child. This case report describes 42 years old multipara admitted to the maternity ward of the Virovitica hospital because of the headache, vaginal bleeding and high blood pressure (170/110 mmHg) in the 37th week of pregnancy who was not regulary controlled. In spite of the antihipertensive therapy the blood pressure was continuosly rising (190/120 mmHg). Considering preeclampsia and suspect placental abruption, we decided to urgent termination of pregnancy by cesarean section. After induction in general anesthesia the intubation was unsuccessful (Mallampati IV), she vomited the gastric content. Another attempt to intubate failed, oxygen saturation dropped and signs of aspiration appeared. The attempt of spinal anesthesia also failed and another general anesthesia likewise. CTG occasionaly showed fetal bradycardia. Finaly it was decided to do the urgent tracheostomy and the patient's hemodynamic parameters stabilised. A fairly asphyctic newborn was delivered by cesarean section, intubated and reanimated. The mother was transfered to intensive care unit where further intensive therapy measures were undertaken with the aim of stabilisation of the patient and treatment of respiratory complications. Sixteen days after, both mother and the newborn left the hospital in a fairly good condition.
