Sažetak

Over the last decades, the reduction of lowdensity lipoprotein cholesterol (LDL-C) with statins has become a part of the standard treatment regimen in patients with and at risk for coronary artery disease (CAD), and thus has resulted in a major advance in pharmacotherapy for the prevention of cardiovascular (CV) events. The greatest reduction in adverse CV events has been reported in trials with the greatest LDL-C reduction, where an approximately linear relationship between LDL-C reduction and clinical event prevention has been observed; for every 1% reduction in serum LDL-C, the relative risks of CV events have been reduced by 1%. In recent trials greater benefits have been reported with more intensive statin regimens than with those moderate, particularly in patients with acute coronary syndromes and high risk patients with stable CAD. The beneficial effects of intensive statin therapy are additionally amplified by a dose-dependent activation of cholesterol- independent - pleiotropic – effects. Despite this evidence, however, some estimates suggest that fewer than 50% of the patients who would benefit from statins are taking them. Most of the treated patients receive statins in inappropriate, low, doses. The potential underlying reasons include the lack of knowledge or the failure to implement the guidelines for primary and secondary CV disease prevention, poor compliance with lipid-lowering therapies, and fear of potential side effects. Therefore, the aims of this review are to present: 1) pharmacological properties of statins, 2) therapy benefits on the basis of the results of clinical studies, 3) target values of serum lipids in patients with and at higher risk of CAD, and finally, 4) possible side effects.
