Sažetak

Aim. Determining the frequency of hypertensive reaction (HR) during the exercise test in optimum, normal and high normal blood pressure (BP) and the first degree of arterial hypertension (AH) and determining the impact of antihypertensive therapy on HR occurrence.
Methods. Patients were tested in the ergometric laboratory of Karlovac General Hospital in the period from 1 June 2010 to 31 March 2011. We examined 859 patients. The age range was between 18 and 80 years, and the mean age was 62.4 years. Subjects were divided according to BP before loading into two groups. The control group were patients with optimal BP (< 120/80 mmHg), while the experimental group were those with normal (120-129/80-84 mmHg) and high normal BP (130-139/85-89 mmHg) and first
degree of AH (140-159/90-99 mmHg). The patients were divided according to their antihypertensive therapy. The patients were followed in hypertensive reaction development in the test load (> 180/120 mmHg).
Results. In 32 subjects with normal BP without antihypertensive therapy and 191 patients with antihypertensive therapy, the HR was not significantly higher compared to patients with optimal BP. In 20
patients with high normal BP without antihypertensive therapy there was no more HR in comparison to patients with optimal BP, but there was more HR in 157 patients with antihypertensive therapy (Z = 1.853, p < 0.01 ). In the first degree of AH, in 34 patients without antihypertensive therapy (Z = 1.562, p < 0.01) and in 238 patients with antihypertensive therapy (Z = 4.002, p < 0.001) there were more HR than in patients with optimal BP.
Conclusion. Given the increase of HR in load, RR would be needed to maintain the preload values optimal or normal.
