Sažetak
Three well-defi ned autoimmune diseases that
affect the liver are autoimmune hepatitis (AIH), primary biliary
cirrhosis (PBC) and primary sclerosing cholangitis (PSC). These
disorders are called ‘‘autoimmune’’, because non-organ specifi c
antibodies can be found in patients’ serum. In patients with AIH,
the main target is hepatocyte. In PBC and PSC, the main targets
are bile duct cells. Diagnosis of autoimmune liver diseases is
based on the combination of typical clinical, serological and
histological features. Overlap syndromes have been described
in patients where this kind of differentiation is less evident. In
addition, some patients with chronic viral liver infection may
develop an autoimmune reaction, the best example being the
presence of autoantibodies in patients with chronic hepatitis
C and the aggravation of the disease when a patient receives
treatment with corticosteroids. Despite the complexity and
incomplete understanding of pathogenesis of autoimmune liver
diseases, the majority of patients respond well to corticosteroid
therapy (AIH) or therapy with ursodeoxycholic acid (PBC or
PSC).
