Caesarean scar pregnancy: case report

Sažetak

Aim: To report a case of a patient with caesarean scar pregnancy, describe the diagnostic and therapeutic course and analyze used and other recommended methods. Case report: A 40-year-old female patient was examined by her gynecologist to evaluate pregnancy, but the transvaginal sonography lead to suspicion of cervical ectopic pregnancy, therefore she was admitted for further observation. A reevaluation of the sonography showed a gestational sac close to the scar of the previous cesarean section. It was decided that due to a high risk of possible complications, including massive hemorrhage and uterine rupture, an abortion was required. Administration of mifepristone and misoprostole was attempted with no effect. Consequently, a hysteroscopic evacuation of the pregnancy was performed successfully without any post-operative complications. Conclusion: Caesarean scar pregnancy should be considered a life threatening condition. The diagnostic approach should include a transvaginal sonography and the therapeutic approach could either include a medical abortion with mifepristone and misoprostole protocol or methotrexate, or a surgical approach by hysteroscopy or laparoscopy. To conclude, the main aim is to terminate the pregnancy by abortion (medical or surgical) while preserving fertility and good quality of life after the treatment.
