Sažetak
Chronic obstructive pulmonary disease (COPD) is the third leading cause of death with a share of about 6% of all deaths in the world and about three million deaths per year. COPD is characterized by a progressive course and great heterogeneity of clinical features. An essential characteristic of COPD is the systemic inflammatory effect, which greatly contributes to the frequency of numerous associated conditions and diseases, the so-called comorbidities that complicate and contribute to the mortality of patients with COPD. Acute exacerbations of COPD (AECOPD) have an extremely unfavorable effect on the course and outcome of the disease, as well as on the risk of death. AECOPD are a frequent reason for visits to the emergency department and hospitalization. Given that patients with frequent exacerbations are particularly at risk, they should be given clear recommendations after treatment in the emergency department to prevent future exacerbations. Mild and moderate AECOPD are treated in the emergency department with the use of bronchodilators, corticosteroids, antibiotics and oxygen therapy. In severe AECOPD, early administration of high-flow oxygen via nasal cannula and noninvasive mechanical ventilation greatly reduces the need for invasive mechanical ventilation and admission to the Intensive Care Unit. The goal of these guidelines is to create a standardized approach to the treatment of COPD in the emergency department as well as standardized written recommendations at discharge from the emergency department in order to harmonize and improve the treatment of COPD at all levels of health care.
