[Microtrauma of the oral mucosa and the location of lesions in chronic vulgar pemphigus]

Abstract
On basis of an analysis of the location of fairly persistent oral lesions or of those which occur sporadically, when the tendency to development of bullae on the skin has quite disappeared under effect of treatment, we consider frequent and varied microtrauma to the epithelium of the oral mucosa as responsible for their location in the oral cavity. By the intensity of their effect these microtraumata are far more numerous and severe tham are the perlistat'ic factors which involve the skin only. In addition to the other therapeutic measures in the tretament of chronic vulgar pemphigus in the oral cavity it is necessary that all — even the slightest traumata be avoided or reduced to a minimum. This does not refer only to the mechanical irritation due to sharp-edged or carious teeth or prostheses but also to physiological lesions to the highly vulnerable mucosa in the oral cavity, lesions which include microtraumata occurring during mastication and probably also at speaking. It would be necessary to study in larger material whether the concept of a higher treshold of irritability to the Körbner phenomenom in the oral cavity or of the activity of the saprophytic apathogenic flora in the oral cavity should be adopted.
