Effect of anatomic pillow on pain, quality of sleep and global health in patients with neck pain

Abstract

Neck pain is one of the most common global health problems because it is connected with patient’s disability and lack of work capacity. Neck pain is treated with analgesics, non-steroidal anti-inflammatory drugs, tricyclic antidepressants, correct posture and stress control education, physical therapy, therapeutic exercises and in some cases using neck collars.
The aim of this single-blinded „sham” controlled clinical study was to evaluate the effect of a foam-filled contour and individually chosen anatomic pillow (ORTOSAN) on pain, global health and quality of sleep in patients with neck pain.
The study included thirty consecutive patients having neck pain who were randomly allocated into two equally numbered groups. To all subjects therapeutic ultrasound (10 sessions) was applied on paravertebral muscles of the cervico-scapular region and they performed supervised therapeutic exercise, too. The patients in Group 1 were given an individually-sized anatomic pillow to use, while patients in Group 2 used their usual pillow. Baseline evaluation included: pain intensity, global patient’s health assessment (GPtA), global examiner’s health assessment (GEA), quality of sleep and awakening due to neck stiffness and/or pain in neck/hand and disability index due to neck pain, the Neck Disability Index (NDI).
After one month subjects in Group 1 had better results comparing to Group 2 in GPtA (P = 0.02), GEA (P = 0.01) and quality of sleep (P = 0.04), as well in awakening due to pain/stifness. Statistically significant improvement in many variables of NDI was found in subjects sleeping on anatomic pillow.
In conclusion, in our sample of ambulatory middle-aged patients with neck pain (with or without radicular symptoms) use of contour individually-sized anatomic pillow (ORTOSAN) resulted in significantly more improvement in various observed parameters comparing to patients sleeping on their own pillow.
