Sinus node dysfunction in patients with acute myocardial infraction

Abstract
We have studied 124 patients (89 men and 35 women) with an acute myocardial infarction. Sixty-six patients (53 %), 28 women (80 %) and 34 (42 %) men had sinus node dysfunction. Sinus tachycardia occured in 43 patients (34,6 %), sinus bradycardia in 13 (40,4 %) and atrial fibrillation in 9 patients (7,2 %). In 59 % of the patients a sinus node dysfunction developed during the first 24 hours in the Coronary Care Unit. Sinus tachycardia was found in 25 patients (75,7%) with anterior myocardial infarction, in 12 patients (44,4%) with posterior infarction and in 6 patients (100%) with subendocardial infarction. Sinus bradycardia was found in 9 patients (33,3 %) with posterior infarction and in 4 patients (12,2 %) with anterior infarction (p < 0,01). The mean peak value of CPK in patients with sinus tachycardia was 501 ±623 U/L, with sinus bradycardia 246 ±223 U/L and with atrial fibrillation 448 ±300 U/L (p < 0,05). Sinus node dysfunction was found in 15 out of 18 patients (83 %) with left ventricular pump failure.
