Supraventricular l tachycardia in a neonate as the cause of cardiogenic shock: a case report

Abstract

Aim: to present a 16 day old neonate who suffered cardiogenic shock due to prolonged attack of supraventricular tachycardia. Case report: A 16-day-old male neonate was admitted to the emergency room due to poor feeding and poor general condition. At admission, he was lethargic with normal blood pressure, low saturation, dyspnoea, cold skin, heart rate was 250 per minute, prolonged capillary refill time and weak pulses. Heart action was rhytmic with quiet heart sounds. Pulmonary auscultation showed basal pulmonary crackles. The liver was palpable for 6cm and the spleen for 4cm. Electrocardiogram revealed supraventricular tachycardia (HR 285 bpm which was successfully terminated with synchronized cardioversion (1 J/kg). Heart ultrasound showed normal heart structure, heavily impaired contractility, mitral and tricuspid valve regurgitation. Laboratory tests leucocytosis, increased troponin and lactate values. The diagnosis of supraventricular tachycardia has been set. Conclusion: Cardiogenic shock is a rare condition in children. Clinical presentation varies according to child`s age and underlying etiology. In treatment of CS we use intesive treatment measures (A, B, C, D, E approach). The recognition of early signs and underlying etiology of CS together with intensive care treatment are the key points in curing these patients.
