COVID-19 pandemic The need to treat a large number of patients with acute respiratory failure

Abstract

Through the first three pandemic waves, Clinical Hospital Dubrava was the Primary Respiratory Center and cared for almost all patients from the Zagreb area. The advantages of central care are simpler supply, easier implementation of a unique therapeutic approach, easier change of experience, and accelerated learning and acquisition of experience, reducing the risk of hospital transmission of the virus. Weaknesses of the system: the loss of the hospital facilities for other patients who become collateral victims of the pandemic, the established gas network easily becomes insufficient due to technical problems, more frequent staff fatigue and “burnout” due to the feeling of hopelessness. Until January 2022 results of the work of the Primary Respiratory Center: mechanically ventilated:1 110, NIV (non invasive ventilation) 610, HF(high flow) 1,860 patients and 32% of patients
needed more intensive respiratory support. The daily consumption of oxygen rose to 360,000 liters of liquid oxygen, six intensive care units with 84 ventilators, an High flow department with 40 beds were formed. HF was also performed in wards where sufficient oxygen pressure could be achieved. The COVID-19 pandemic has put us in a situation where we need to take care of a large number of patients with respiratory problems. Long-term planning and development of measures are needed to ensure the best possible conditions for the care of these patients by improving the medical, technical and personnel prerequisites for successfully dealing with this problem.
