Abstract

Diabetes mellitus is the most common cause of chronic renal failure in the world.
There appears to be different pathogenetic, hemodynamic and metabolic processes leading
to the pathologic mechanisms in diabetic nephropathy. In recent years, great progress has
been made in understanding the risk factors and pathogenetic processes in diabetic nephropathy.
In addition, new insight into the pathophysiology of this chronic disease has lead to
better preventive therapies, treatment possibilities and reduced progression to end stage
renal disease.
This review aims to give an overview of the pathology, diagnosis and treatment possibilities
of diabetic nephropathy. We give insights into the mechanisms leading to pathological
changes in the kidney and clinical parameters associated with these alterations, which allow
reliable characterization of the stages of kidney disease and the subsequent choice of treatment
strategy. Furthermore, we describe recent developments in the prevention and treatment
of diabetic nephropathy targeting the main risk factors, resulting in renal protection
and reduced disease progression. Finally, we address how treatment of diabetic nephropathy
affects other diabetes associated diseases, and how it reduces overall and cardiovascular
mortality in diabetic patients
