CLINICAL EVALUATION OF THE IMPACT OF MEDICAL EXERCISES ON PAIN REDUCTION AND PHYSICAL ABILITY IN PATIENTS WITH KNEE OSTEOARTHRITIS

Sažetak

INTRODUCTION: Osteoarthritis is the leading cause of disability in older adulthood and tends to become the most common form of musculoskeletal
disease. Within the osteoarthritis group, knee osteoarthritis occurs as the most common entity. Guidelines for the treatment of knee osteoarthritis indicate the importance of non-pharmacological treatment, which includes active medical exercises, in pain relief, increasing physical fitness and preventing or slowing the progression of the disease.
AIM: To show the impact of conducting a program of medical exercises on pain reduction, physical ability and degree of functional disability in patients
diagnosed with knee osteoarthritis.
MATERIALS AND METHODS: On a sample of 92 subjects diagnosed with knee osteoarthritis, active medical exercises with a progressive increase in
load were performed for 3 months, five days a week under the supervision of a physiotherapist. A clinical evaluation was performed with the visual analogue pain scale (VAS), Health assessment questionnaire (HAQ), Short physical performance battery (SPPB), and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Also, an analysis of changes in body mass index and bone and muscle mass was performed using bioimpedance analysis. Statistical processing includes a t-test for related samples with respect to the normal distribution of data within the group.
RESULTS: Statistical analysis objectified, statistically significant, decrease in pain in VAS (95% CI -1,05 (±1,79), p<0,001), decrease in WOMAC index
(4,73-(-1,96), p<0,001) and HAQ (-0,18-(-0,02), p=0,012) and increase in SPPB composite test (95% CI 0,02-0,44, p=0,034), which correlates with the results of Cochrane analysis on the impact of medical exercise on knee osteoarthritis. There was no statistically significant change in the increase in body mass index, muscle and bone mass based on bioimpedance analysis of body composition.
CONCLUSION: Based on the conducted research, a statistically significant impact of medical exercises on pain reduction and their statistical significance
on physical fitness and self-assessment of the patient's health, in patients diagnosed with osteoarthritis of the knee, immediately after the 3-month
medical exercise program was shown. Medical exercises represent an indispensable non-pharmacological therapeutic modality in the treatment and prevention of the progression of knee osteoarthritis in patients who have no contraindications for its implementation.
