Sažetak
Accidental hypothermia is defined as an unintentional fall in core body temperature to less than 35°C. The principles of initial management of hypothermia depend on the clinical stage. Minimally-invasive rewarming for patients with primary hypothermia who are not in cardiac arrest and who have stable vital signs and have no comorbidities, can substantially enhance their survival. Extracorporeal life support (ECLS) has advanced role in the management of pateints in hypothermic cardiac arrest, with survival rates approaching 100% in some cases. Hypothermic patients with risk factors for imminent cardiac arrest (temperature <28°C, ventricular arrhythmia, systolic blood pressure <90mmHg), and those who are already in cardiac arrest, should be transferred directly to an ECLS-centre. Cardiac arrest patients should receive continuous cardiopulmonary resuscitation (CPR) during transfer. If prolonged transport is required, mechanical CPR can be helpful. Delayed or intermittent CPR may be appropriate in hypothermic arrest when continuous CPR is impossible. The purpose of these recommendations is to help clinicians in diagnosing and treating of accidental hypothermia in the emergency medicine using structured recommendations.
