Sažetak

Chronic noncommunicable diseases, especially cardiovascular diseases, type 2 diabetes, dyslipidemia and obesity, are the leading causes of mortality in Croatia. Their management requires lifelong therapy and a multidisciplinary approach. Community pharmacists, as the most accessible healthcare professionals, play an increasingly important role in prevention, early detection, and ongoing care of chronic disease patients. This article provides an overview of the current treatment guidelines for arterial hypertension, type 2 diabetes, dyslipidemia, and obesity, with special emphasis on novel therapeutic options and the pharmacist’s role within the healthcare team. Special attention is given to medication adherence, the importance of empathy in patient communication, and the role of patient education and outcome monitoring in community pharmacies. The year 2025 has been declared the Year of Pharmacy in Croatia, offering a unique opportunity to further promote the pharmacist’s role in chronic disease care.
