Contemporary management of urolithiasis: experiences from Department of urology, Clinical hospital center Osijek

Abstract

PURPOSE: “Minimal invasive” techniques of urolithiasis management have been established at Department of Urology University Hospital Osijek since 1986. - 1988. From 01.July 1988. to 31-December 2009. 10570 patients underwent procedures of extracorporeal shock-wave lithotripsy (ESWL), percutaneous lithotripsy (PCNL) or ureterorenoscopy (URS) for urinary stones. The aim of the study was to evaluate indications, features, results, ancillary procedures and complications related to the ESWL, PCNL and URS treatment for urinary stones at Urology Department of University Hospital Osijek.
PATIENTS AND METHODS: At analised period 5831 men, 4976 women and 94 children aged 2 to 79 years (average age 42.23 years) were treated. They underwent a total of 11305 ESWL, 262 PCNL and 623 URS treatments. Success rate of a treatment was evaluated 3 months after the last hospitalization, considering plain film, urinary tract sonography, and laboratory (biochemistry and microbiology) findings.
RESULTS: After retrograde evaluation of available medical records of 5542 patients a total of 73,83% of ESWL treated patients was considered as “stone free”. The staghorn stones were treated by ESWL monotherapy with success rate of 59.02%. There was total of 132 staghorn stones treated by PCNL alone (success rate 75.28%) and 130 “sandwich” treatments (PCNL + ESWL) with success rate of 82.61%. URS have been applied after foil of ESWL at 381 (61.16%) and at 242 (38,84%) of patients as a first choice method.
CONCLUSIONS: Early after establishing of Osijek Civilian Hospital at far 1874 we can found continuous written traces by Osijek s physics, surgeons and finally urologists of their efforts in contemporary management of urolithiasis. However, the final step was achieved in 1986-88 by successfully establishing a “minimal invasive” methods of ESWL, PCNL and URS just a few years after they entered a clinical practice at leading urological countries.
