Sažetak

As an important complication of acute pyelonephritis, kidney abscess requires prolonged antimicrobial treatment, often with percutaneous drainage. Due to non-specific symptoms, including blood and urine tests, kidney abscess should be considered in the differential diagnosis in a patient with acute pyelonephritis showing slow response or failure to antimicrobial treatment. Here we present a case of an 86-year-old female patient without any chronic diseases or history of urinary infections, who was admitted with a diagnosis of acute pyelonephritis, which was complicated by intrarenal abscess and bacteriemia caused by Escherichia coli producing extended-spectrum beta-lactamase. Radiologic detection of a kidney abscess was crucial to establishing a definitive diagnosis, as well as antimicrobial susceptibility results, which showed resistance to the initially started therapy. The patient underwent prolonged treatment with a complete resolution of symptoms and radiologic signs of abscess on clinical follow-up.
