Extracorporeal Membrane Oxygenation in the Treatment of Acute Respiratory Distress Syndrome Induced by Pandemic H1N1 Influenza Virus

During the H1N1 influenza pandemic in the end of 2009 and the beginning of 2010, intensive care units (ICU) throughout the world faced a new clinical entity – primary influenza pneumonia. In the most severely ill patients, influenza pneumonia was clinically manifested as acute respiratory distress syndrome (ARDS). Conventional mechanical ventilation frequently failed to achieve adequate oxygenation in these patients. Consequently, new treatment strategies were urgently explored to enhance the survival of the patients with the most severe form of ARDS induced by the H1N1 pandemic influenza. In those circumstances the University Hospital for Infectious diseases “Dr. Fran Mihaljević” obtained two machines for extracorporeal membrane oxygenation (ECMO) with the support from the Ministry of Health and Social Welfare. After a two-year experience with ECMO acquired during the influenza pandemic, the patients with the most severe form of ARDS in Croatia have a new treatment option and an improved chance of survival regardless of the ARDS etiology.
