PRIMARY PSOAS ABSCESS IN CHILDREN

Sažetak

Psoas absces is a rare pathologic condition in children, often with atypical course and clinical picture, resulting in delays in diagnosis and significant morbidity. There are two types of psoas abscess: primary psoas abscess and secondary psoas abscess. While the cause of primary psoas abscess remains unknown, secondary psoas abscess results from the spread of infection from the environment or due to another pathologic condition. Traditionally, psoas abscess has been usually brought in connection with tuberculosis cold abscess, consequential to tuberculous caries of thoracic and lumbar spine. Pain in lower abdomen, painful hips and limping with fever are common symptoms. The most frequently isolated causative agent is Staphylococcus aureus with elevated inflammatory parameters (erythrocyte sedimentation rate, leukocytosis). Diagnosis is made by ultrasound and computerized tomography. Extraperitoneal drainage with antibiotic therapy leads to rapid recovery and cure in the majority of patients. In this paper, we point to a rare pathologic condition and indicate how to reach an accurate diagnosis and treatment of such diseases.
