Sažetak

Proliferative lesions of specialized prostatic stroma are rare with only isolated case reports documented in literature. These lesions are histologically characterized by exuberant proliferation of prostatic stroma with variable number of normal and hyperplastic glands. Recently this group of tumors has been labeled as prostatic stromal proliferation of uncertain malignant potential (PSPUMP). The behavior of this lesion cannot be predicted by its histological appearance.
We report a case of PSPUMP in a 71-year-old male who underwent transvesical adenectomy for a urinary obstruction. Based on histological appearance and immunohistochemistry, a diagnosis of PSPUMP was made. After a 5-year follow up, the patient has not shown any evidence of recurrence or progression to malignant prostatic stromal sarcoma.
The main differential diagnosis includes prostatic stromal sarcoma, leiomyosarcoma and rabdomyosarcoma.
In view of the risk of recurrence and possibility of progression to frankly malignant prostatic stromal sarcoma, a long – term follow up will be required for patients with this diagnosis.
