Sažetak

The painful temporomandibular disfunction is discussed from the aspect of ist relation to the entire pathology of the temporomandibular joint, especially to the myofascial painful syndrome and Costen’s syndrome. These syndromes are nowadays mentioned in the literature as different pathophysiological conditions of a similar but nevertheless different symptomatology which should be differentiated in order to be able to choose the best treatment. The author however thinks that in the very complex relationship between the temporomandibular joint, masticatory mucles, jaws and teeth, they should not be differentiated as they are pathophysiologically connected; a disfunction can provoke the myofascial pain, and vice-versa the myofascial pain can be the cause of the disfunction. Costen’s syndrome is most probably only a specific picture in a sequence of possible deficiency of the temporomandibular joint, painful disfunction and the myofascial »trigger« mechanism.
