Precancerous conditions of the oral mucosa and their prevention

Sažetak
Precancerous conditions of the oral mucosa are a fairly well defined concept in view of the pathohistological substrate of the lesions, but there is a certain vagueness with regard to the clinical diagnostics of a number of changes which are only potentially cancerogenic. Some changes, such as e. g. malignant leucoplakia melanosis circumscripta praecancerosa, erythroplasia Queyrat and Morbus Bowen may be considered as premalignant lesions since histopathology may it confirm. The dynamicsof hereditary factors in their appearance has not yet been entirely cleared. Reversely, some precancerous conditions are only potentially metaplastic whose actual histo-pathological substrate does not as yet show any essential deviations in the sense of canceration. Therefore the collective term »Predisposition of the oral mucosa to the development of carcinoma« is a clinical term only and does not necessarily indicate malignant alternation. Such afflictions are for example Lichen ruber Pemphigus Vulgaris, Plummer-Vinson's syndrome and some others. A separate group of precancerous conditions consists of pathological changes of the oral mucosa which are the result of mechanical or chemical irritations. Their chronic persistence develops them into malignant lesions under so far unknown conditions. Such changes are the ulceration of the edge of the prosthesis chronic cheilitis, galvanism of the oral mucosa and the like. The effect of chemical nutritional cancerogenic substances on the mucosa of the oral cavity has so far not been studied in detail. The role of the stomatologist in detecting, diagnosing and prescribing treatment for these precancerous conditions is quite significant.
