[The possibility of surgical treatment of extensive periapical processes]

Sažetak
The author reports on some cases o f teeth with periapical ostitis which affected more than the apical third of the root. The teeth were moreover clinically relatively mobile with a more or less widened parodontal gap. Endodontic treatment is combined with the method of fixing the teeth with a wire appliance reinforced with acrylic and subsequently performing surgery. Amputation of the apical part and removal of the granulation or the cystic sheath was undertaken. The root of the tooth remained partly protruding in to the bony defect. This space was filled by later following osteogenesis and the teeth were firmly secured. The wire appliance reinforced with acrylic ensures undisturbed rest of the amputated root of the tooth during the period in which the bony trabeculae begin to form . Control examinations after 4, 8 and 12 months showed a normal postoperative course though we had no occassion to observe these teeth over a number of years. Still it is considered that a good prognosis may be predicted from the results achieved so far.
