Salmonella enteritidis as a cause of pneumonia and pleural empyema in immunocompetent patient

Sažetak
Salmonella enteritidis belongs to the group of nontyphoidal (NT) Salmonella which are primarily animal parasites and only occasionally infect humans causing mostly acute gastroenteritis. Invasive disease and extraintestinal manifestations occur in about 3.3% of NT Salmonella infections. Pleuropulmonary NT Salmonella infections are a rarity and mostly occur in immunocompromised patients. In September 2014, a 53-year-old man was admitted with clinical, laboratory and radiological features of acute bacterial pneumonia accompanied by diarrhea and vomiting. Xpect™ Legionella immunochromatographic urine antigen test for the presence of Legionella pneumophila antigen in urine came positive so the treatment with moxifloxacin was started. Eventually, pleural empyema was detected by computed tomography and Salmonella enteritidis was cultured from pleural exudate. Thoracotomy, decortication of affected lung and resection of necrotic lung tissue was performed. Serological testing of "paired sera" was negative for Legionella pneumophilla thus excluding the bacteria as a cause of pneumonia and empyema so Xpect™ Legionella test was interpreted as false positive. NT Salmonella are a possible cause of pleuropulmonary infection in an immunocompetent patient. Caution is warranted for interpreting positive urinary antigen tests for legionellosis in patients with pneumonia and symptoms of acute gastroenteritis.
