Abstract

Neuralgia is a chronic peripheral neuropathic painful syndrome that occurs as a consequence of infection by herpes zoster virus. After herpes virus infection about 10-20% of patients develop post-herpetic neuralgia with persistent neuropathic pain. Its incidence increases with age and the increasing incidence of diseases that weakened the immune system. Postherpetic neuralgia is a disease of adults and elderly. Neuropathic pain is an unpleasant sensory and emotional experience caused by damage of the somatosensory nervous system. The pain occurs spontaneously, especially during the sleep, and is perceived as burning, stinging, throbbing or sharp pricking pain. Pain disrupts sleep, causing anxiety and depression and reduces quality of life. This paper presents the patient with postherpeutic neuralgia. Two objectives needed to be achieved in the treatment of post-herpetic pain of the patient : to establish the appropriate diagnosis and treatment of the underlying disease and to diagnose the pain syndrome and provide symptomatic pain treatment. A combination therapy with opioid analgesic, topical analgesic, capsaicin and venlafaxine was used in the treatment of pain. Diagnosis and treatment of neuropathic pain require an interdisciplinary and multimodal approach. Therefore the patient was finally treated with acupuncture, which ultimately had the best analgesic effect. Psychotherapeutic support was extremely important in the treatment of chronic pain in the patient.
