Fibrobronchoscopy in the etiological diagnosis of pneumonia

The paper discusses 30 patients suffering from alveolar pneumonia, who prior to hospitalisation did not receive any antibiotic therapy. The aim was to compare the results of sputum antibiograms to antibiograms of bronchial secretion obtained by means of fibrobronchoscopy and to evaluate adventages of the latter method. The purpose of the study was to determine the etiological diagnosis of pneumonia and apply the appropriate therapy. Sputum was taken by the usual method and bronchial secretion by means of an ordinary catheter during fibrobronchoscopy. A significant difference was established between antibiogram results. Normal flora was isolated in 50% of sputum samples and 16.6 % bronchial secretions. Not one Streptococcus pneumoniae was found in bronchial secretion samples; on the contrary, Gram-negative flora was dominant. In 33% of cases Pseudomonas aeruginosa was isolated, which does not correspond to the literature nor to the course of the disease and the response to antibiotic therapy. Fibrobronchoscopy enables obtaining of clinical material from the site of the pathological process and has advantages, according to bronchial secretion analysis. However, the interpretation of antibogram and the decision on therapy depend very much on the expertise and experience of the clinician.
