Influence of several psychotropic drugs on blood pressure

Changes in blood pressure under the influence of psychotropic drugs are frequent occurence in psychiatric wards. The aim of the study was to test the influence of psychotropic drugs on patients’ blood pressure by a procedure which would allow a follow-up of changes, give data about the frequency and about seriousness of this side-effect as well as indicate the possibility of its prevention. Schellong’s test of orthostatic tolerance seemed the most appropriate method. Patients were classified into five groups according to the psychotropic drugs they were taking: Diazepam, Maprotilin, Levomepromazin, a combination of Levomepromazin and Maprotilin and Fluphenazin. The orthostatic changes of the blood pressure were estimated by Schellong’s test before the treatment and at the 1st, 4th, 8th, 11th, 15th, 18th,22nd and 25th day during the treatment. The results obtained confirmed the clinical significance of the orthostatic hypotension. It was found at least once in 49 per cent in the case tested and its symptoms in 45 per cent. Twelve patients collapsed. Levomepromazin and its combination with Maprotilin provoked very frequent and severe hypotension showing the greatest hypotensive power. The author concluded that changes in blood pressure are very important clinical side-effects we should think of when applicating psychotropic drugs. It is especially important during the first two weeks. The great variety of this side-effect deserves caution even when using psychotropic drugs of small hypotensive power. Schellong’s test applied before and during the treatment fullfilled the need of clinical estimation of the pharmacogenic orthostatic hypotension.
