Sažetak

Complex therapeutic regimes and potential adverse eUects underline the need for interventions that will increase patient adherence and the safety of medicines thus reducing the risk of drug-related problems. Collaborative practice between pharmacists and primary care physicians can play a major role in optimising a patient's pharmacotherapy. The main objective of this interventional study was to identify and describe the number and type of pharmaceutical interventions made during the collaborative practice with primary care physicians. The study was conducted at a family practice in Split and included 92 patients (mean age 67.5 (± 13.9), with 70.7% of subjects being women). By analysing relevant patient information obtained from the medical documentation provided by the physician, and from the consultation with the patient, the pharmacist identi[ed the drug-related problems and made appropriate interventions. These interventions included therapy-related advice to the patient and recommendations to the physician. During the one-month period of collaborative practice, the primary care physician made changes in drug therapy based on pharmacist's recommendations in 46 cases (by replacing the drug, modifying the dosage regimen and monitoring the patients more closely), while the pharmacist intervened in additional 48 cases by consulting the patient (providing advice related to non-prescription and proper administration of prescription medicines, as well as highlighting the importance of adherence to a drug regimen). In conclusion, the study emphasized the importance of physician-pharmacist collaborative practice which has helped identify and reduce drug-related problems, as well as optimise patients' pharmacotherapy.
