Assessment of functional status in patients with severe aortic stenosis after transcatheter aortic valve implantation

Sažetak

Introduction: Aortic stenosis is the most common valvular heart disease in people older than 65 years. Patients who develop a severe degree of this disease have marked symptoms with even the slightest exertion, and their functional status is impaired. Since drug treatment continues to have little success, replacement of the aortic valve is the most important step in the treatment of symptomatic patients.
Objective: The objective od this research is to assess the functional status of patients before and after the use of TAVI according to the NYHA classification.
Methods: The study was retrospective and was performed on 60 patients who were treated for severe aortic stenosis at University Hospital Center Rijeka in the period from 01/01/2016 to 01/01/2020 and in whom the Structural heart disease team determined the need for TAVI surgery. The functional status of the patients was assessed using the Functional Status Assessment Scale (NYHA classification) before and after the procedure, as were the echocardiographic findings of all patients included in the study. Data were processed using descriptive and inferential statistics.
Results: Of the total 60 patients (100%), 31 women (52%) and 29 men (48%) participated; the mean age of men was 78.1 ± 7.3 years and that of women was 80.55 ± 4.43 years. Subjects had a NYHA classification level of 2-4 before the procedure, while after the procedure the first ejection fraction before the TAVI procedure in men was 45%-60%, after the procedure it was 47.5%-60%, and in women the ejection fraction the fraction before the procedure was 50%-60%, and after the procedure 55%-60%.
Conclusion: Patients with severe aortic stenosis treated at Clinical Hospital Center Rijeka have improved functional status according to NYHA classification after the use of TAVI in both sexes, regardless of age.
