Habitual and psychogenic cough – new nomenclature

Sažetak

Cough is a protective reflex that prevents aspiration of contents into the respiratory tract and improves its cleaning. An effective cough is necessary to maintain the health of the respiratory system, and in practice it is one of the most common symptoms with a significant socioeconomic burden.1 Excessive and prolonged coughing, especially when it is more pronounced, significantly reduces the quality of life.2,3 Regardless of whether the cough is acute4 or chronic5 in children, it has a great impact on sleep, school performance and play. Cough in children presents significantly differently compared to adult cough and its etiology is different.2 Long-term (chronic) cough in children differs from that in adults due to the different morphology of the airways, a higher degree of sensitivity to harmful stimuli, reduced control of the cough reflex and differences in the maturation of the neurological and immune systems in different pediatric age groups.2 There are many causes of chronic cough – most originate in the lungs, but extrapulmonary causes are also present. Also, most causes of cough can be discovered, but some causes are still controversial. Various terms are used to describe cases of chronic cough without a clear pulmonary or extrapulmonary etiology in the presence of some suggestive clinical characteristic and/or association with a psychological problem, including habitual cough, psychogenic cough, and tic-cough.6 In view of the terminological inconsistency and
the need for harmonization with the DSM-5 (The Diagnostic and Statistical Manual of Mental Disorders) disease classification, in 2015 the Expert Group of CHEST (American College of Chest Physicians) made recommendations for the definition of these subgroups of chronic cough that a habitual cough is called a “tic cough”, and a psychogenic cough is called “somatic cough disorder”.6 ERS (European Respiratory Society) accepted the nomenclature of CHEST.
