Dupuytren's Contracture - Viking's Disease in the Modern World

Abstract
Dupuytren's contracture (DK) is a well known, common benign fibromatosis of the hand characterized by a chronic progressive fibrosis of the palmar fascia with flexion deformities of the fingers. This idiopathic disease, known from the Viking age, has a prevalence of 8% of the world population. It is more common in men and heredity plays an important role in its etiology, showing a variable expression. The pathogenesis of DK is still not completely known, but it includes the formation of the collagen nodules that progress into fibrous bands at the places of the greatest pressure between the dermis and the fascia. The diagnosis is usually easily established by the clinical examination itself, and the treatment can be either conservative or surgical. Conservative treatment is recommended for patients with milder symptoms and no contracture, while surgical treatment remains the gold standard and is reserved for the patients with limited hand function and/or flexion contracture of the proximal interphalangeal of ≥15 degrees and/or metacarpophalangeal joint of ≥30 degrees. The aim of this review is to present the latest knowledge about Dupuytren's disease, based on the results of systematic reviews and meta-analyses.
