Adherence in Patients with Psychotic Disorders

Sažetak

Adherence in patients with psychotic disorders has a direct influence on treatment outcomes because these disorders require continuous, long term and often life-long antipsychotic therapy. In contrast to somatic patients who mostly have insight into their illness and the need for treatment, in patients with psychotic disorders, insight is most often absent, especially at the beginning of the treatment of an acute episode of the illness. After that period of so-called “compliance”, treatment phases follow in which the patient gains better insight and cooperation, or adherence is achieved when the patient takes an active role in his treatment. In addition to the patient himself, the therapist also plays a role in the therapeutic process of cooperation by choosing an adequate drug and an adequate dose, individual patient approach, respecting all his peculiarities and creating a good therapeutic alliance. In addition to efficacy of the drug, factors that also influence the achievement of good cooperation are tolerability and safety of the drug and avoidance of side effects. Cooperation or adherence is one of the fundamental prerequisites for successful treatment of patients with psychotic disorders.
