The Role of Family Doctors in Treatment and Monitoring of Patients with Psychotic Disorders

Schizophrenia is a chronic psychotic disorder characterized by delusions and hallucinations in the acute phase of the disease. The exact cause of schizophrenia is not known, but it is believed that a number of biological, psychological and social factors participate in its etiopathogenesis. Symptoms of schizophrenia are divided into positive (delusions, hallucinations, thought disorder, disorganized speech, unusual behavior and catatonia) and negative (emotional numbness, alogia, avolition, social isolation and anhedonia). The focus in treatment and follow-up of patients suffering from schizophrenia is placed primarily on the psychological component of the disease and its psychological comorbidities, but there are a number of physical comorbidities of which awareness should be increased. 75% of patients suffer from another chronic disease. The diagnosis is usually made according to ICD-11 and DSM-5 diagnostic criteria. Schizophrenia cannot be completely cured, but it is possible to bring the disease under control and ensure the patient’s normal functioning. Treatment includes drug therapy (antipsychotics) and psychotherapy. Although the treatment and follow-up of patients suffering from schizophrenia requires a multidisciplinary approach, due to the availability and continuity of care and the trust gained, family doctors are often the first to whom patients or family members ask for help. Their task is to evaluate the patient’s condition and success of the treatment, to recognize the mental and physical comorbidities of the disease and start their treatment, but also to educate patients and their family members on the nature of the disease itself, possible precipitating factors, symptoms / signs that require hospitalization of the patient, and treatment and rehabilitation options.
