Sažetak

Over the last few decades, significant progress has been made in asthma treatment, which has yielded a decline in asthma mortality rates. However, it continues to place a significant strain on global healthcare systems as well as patients’ well-being and quality of life. Asthma exacerbations are responsible for most disease complications, and severe exacerbations occur in patients with severe, moderate, but also mild forms of the disease. According to research, asthma is not controlled as well as it should be in the majority of patients, and preventable factors have substantial impact on unfavourable outcomes. A personalized treatment plan, developed with doctor-patient cooperation, reduces the occurrence of exacerbations, the number of emergency room visits and hospitalizations, thus reducing morbidity associated with asthma. It is key to provide written instructions on how the patient can effectively recognize aggravation of the disease, as well as what to do in these cases. The 2019 GINA guidelines recognize the importance of a personalized asthma plan and propose an important change in treatment: short-acting β-agonists are no longer recommended as monotherapy, and are only to be used in combination with inhaled corticosteroids. The same recommendations were given in the last revision of the GINA document. Current therapy recommendation includes a combination of an inhaled corticosteroid and formoterol. A personalized asthma treatment plan is advantageous due to better adherence to the patient’s needs, provides a more active role for patients in disease management, and enables a more precise determination of the severity of the disease.
