Sažetak

The guideline summarizes the currently available evidence on the diagnosis and treatment of patient with syncope. The guidelines were created based on a systematic review of professional literature. Syncope is defined as a transient loss of consciousness due to cerebral hypoperfusion. It is characterized by a quick onset, short duration and complete spontaneous recovery, and is divided into reflex, orthostatic and cardiogenic. The initial evaluation of a patient begins with anamnesis and heteroanamnesis, which, as well as a physical examination including blood pressure measurement in the lying and standing position, and a 12-channel electrocardiogram are of great importance in planning further diagnostics. The goal of evaluating a patient with syncope in the emergency department is to answer three main questions: is there a serious underlying cause that can be identified, what is the risk of a severe or fatal outcome, and should the patient be admitted to hospital. Risk stratification is important for two reasons (1) identification of low-risk patients who can be safely discharged with appropriate recommendations, (2) identification of high-risk patients in whom hospitalization and further diagnostic and therapeutic procedures are indicated. Despite many attempts to introduce standardized tools for risk assessment into clinical practice, their use has not proven more successful than the good clinical judgment of an experienced physician;, hence, the importance of educating healthcare professionals in the field of emergency medicine cannot be overstated.
