Physiotherapy Interventions in Rehabilitation of Congenital Muscular Torticollis – Review of Guidelines and New Scientific Findings

Sažetak

Congenital muscular torticollis is a postural disorder which usually manifests within the first four weeks following the child’s birth. It is characterised by leaning the head to the affected side and rotating the head to the opposite side, and it is a consequence of unilateral shortening and fibrosis of the sternocleidomastoid muscle. The aim of this paper is to review current professional and scientific literature which deals with the methods of physiotherapy evaluation and interventions in children with congenital muscular torticollis, and to propose the most efficient physiotherapy interventions based on the results of such review. All publications reviewed in this paper favour conservative treatment of congenital muscular torticollis, which includes passive stretching of the sternocleidomastoid muscle, advising parents on proper positioning of the child and use of helmet in case of pronounced plagiocephaly. According to current research, manual massage techniques, which may form part of physiotherapy interventions, do not contribute to improved treatment outcomes, since treatment outcomes are the same in children who were subjected only to stretching exercises and those who additionally received manual massage. The duration of physiotherapy and frequency of sessions depend on numerous factors – mainly on the severity of torticollis and the child’s age at the time when physiotherapy started.
