Sažetak

Aim: To present 25 years of experience in treating diaphyseal and diametaphyseal fractures of long bones in children using the Elastic Stable Intramedullary Nailing (ESIN) method, with emphasis on the incidence and types of fractures, and management of complications. Patients and Methods: This retrospective study included 503 paediatric patients aged 3 to 16 years who underwent ESIN fixation at the Department of Paediatric Surgery, University Hospital Centre Rijeka, between 1996 and 2021. Patients were followed for a minimum of two years after surgery. Demographic data, fracture location, length of hospital stay, and type and frequency of complications were evaluated. Results: Boys were more frequently affected (75.9%) than girls (24.1%). The most common fractures involved the forearm (53.5%), followed by the lower leg (21.0%), femur (18.3%), and humerus (7.2%). A total of 35 complications (6.96%) were recorded—26 clinical and 9 radiological. The majority (85.7%) occurred during the first five years of ESIN implementation. The most frequent complications were local skin irritation and mild functional limitations. The average hospital stay was 2.7 days (range 1–9 days). Conclusion: ESIN is a minimally invasive, tissue-sparing, and effective method for treating long bone fractures in children. When indications are properly selected and technical principles are strictly followed, complication rates can be minimized, ensuring excellent functional and rehabilitation outcomes.
