Severe Hypercalcemia Caused by a High-Dose Magistral Vitamin D Preparation: A Case Report

Sažetak

Aim: We report a case of severe hypercalcemia caused by vitamin D overdose following the use of a magistral preparation containing high doses of vitamin D. Case report: The patient presented to the emergency department with marked generalized weakness and dyspnea. Initial laboratory findings revealed significantly elevated serum calcium levels, suppressed parathyroid hormone, and markedly increased 25-hydroxyvitamin D concentrations. A comprehensive diagnostic evaluation excluded other common causes of hypercalcemia, including malignant, granulomatous, and endocrine disorders. The patient was treated with intensive intravenous hydration and zoledronic acid, resulting in a gradual decrease in serum calcium levels and clinical improvement. Subsequent medical history assessment, together with additional information provided by a pharmacist employed at the pharmacy where the preparation was dispensed, confirmed that the product was a magistral preparation compounded according to a prescription and containing very high daily doses of vitamin D. Conclusion: This case highlights the potential risks associated with high-dose vitamin D supplementation, including the use of magistral preparations, and underscores the importance of obtaining a thorough and targeted medication history. Particular emphasis is placed on cautious prescribing of high-dose vitamin D, clear dosing instructions, and appropriate patient monitoring to prevent serious toxic effects.
