Guided Tissue Regeneration in the Reconstructive Periodontal Surgery

Abstract
Principal purpose of the periodontal therapy is complete regeneration
of the supportive tissue o f the tooth tissue. In the conventional conservative and surgical therapy there occur the apical proliferation and migration o f the epithelium along the root surface. The long junction epithelium inhibit the regeneration of the other tissues of the periodontal structures, primarily of the bone and the periodontal ligament. The concept o f guided tissue regeneration (GTR) includes the elimination of prevention of the apical proliferation of the epithelia aiming to allow enough time and space for the bone cells and the periodontal ligament to regenerate. In early eighties Nyman and coworkers had the idea to insert a membrane between the gingiva and the tooth to select direction o f healing o f periodontal tissues and to make the process of regeneration possible. Since then this concept found quite a broad application in therapy and through it represent a significant step towards the full filament of the main goal o f the periodontal therapy, i.e. towards the complete regeneration o f all supportive tissues of the tooth. Guided tissue regeneration survey of the literature shows that attempts were made to elucidate several problems and to invent as good as possible materials and methods of application of the membranes. Many authors indicate the success o f GTR technique in periodontology and achievement in long term maintenance o f the results abtained through periodontal therapy.
