Abstract

A case o f a 53-year-old man who had suffered from bilateral painless parotid gland swelling and hypersalivation during the past 5 years is presented. Needle aspiration biopsy o f the right parotid gland showed features o f a typical benign lymphoepithelial lesion (BLL). No features o f a systemic disease were found. A diagnosis o f Mikulicz's disease was made and a protocol o f close long-term follow-up o f the patient designed. A variety o f etiologic causes in this rare condition and a diagnostic approach to the patient with chronic parotid swelling are presented. Immunopathologic association between BLL, Sjogren's syndrome and evolving malignant lymphoma is also discussed. Advances in the immunohistochemical technique and Southern's blot hybridization technique have allowed an early detection o f monoclonal B-cell proliferation, i. e. malignant B-cell lymphoma, in the salivary gland aspirate.
