Some problems of mandibular osteoradionecrosis

Abstract

The existing problem of osteoradionecrosis has induced the author to consider some of its aspects. The elements determining therapy are the clinical appearance, X-ray, and general condition of the patient. The author puts forward possibilities for the conservative (medicamentous) therapy and possibilities and dilemmas in the application of surgery. Special consideration is given to an aspect of the prophylaxis of necrosis: ithe extraction of teeth or their roots and removal of the metal centure from the radiation field. Preoperative and, particulary, cobalt and betatron radioation is discussed from the point of view of the development of osteoradionecrosis following these kinds of radiation. The author points out the lack of data on processes in the region of the temporomandibular joint in adults after the radiation of tumours in the immediate vicinity of the joint.
